Looking back on the Space Station Freedom project that eventually became ISS
I've been wondering what NASA planned to use as a lifeboat for crews. The
Russians weren't invovled so Soyuz wasn't the option early on. Then I noticed

For the sake of argument, let's say an astronaut conduction an EVA
slipped out of the foot harnessed and started to float away. The only
way to resuce is for the shuttle to undock from the ISS and go get the
astronaut. How quickly could the shuttle do this?

I have a doubt about the first mission of Columbia.
If I am not wrong, during lift off there were a lot of impacts on the
thermal shield (tiles) because of foam debris coming from the external
tank. I think that this caused some damage on tiles, but what I am not
