For awhile there was speculation this was a returning Apollo-era S4B stage,
but more recent measurements seem to lower the chances of that...

Has there ever been a spontaneous and catastrophic failure of a cryo
upper stage during ascent?  All of the Saturn upper stages were
successful, as far as I can see, and I don't see any explosions of
