"Do we do a good job of asking for minority dissenting opinions?  Does
everybody have a voice?  Can you voice your opinion, even if it's not
a popular opinion, without fear of retribution?  If not, what changes
do you think would make a difference?"

The ppt slideshow (hosted by Nasawatch) was distributed at JSC as part
of the post-Columbia corrective action.

It will be interesting to see what non-related organizations learn
from NASA's efforts toward avoiding tragedy.

It will be interesting to see what NASA learns from NASA's efforts.  I
wonder what a "work group" consisted of.  "Natural work groups" seems to
be standard organizational and staff meetings.  Who led the discussions?
 Seems to be the same as it ever was.
