How does one 'man-rate' a rocket?

I was wondering what special testing and such must be done before a rocket
is man-rated. Can any rocket be 'man-rated'? What does the term mean in
terms of quality assurance and such?

There was recently a standard that came out on this topic which I
believe another poster pointed out.

However, traditionally, it means that the risk was evaluated and was
deemed acceptable to the Project Office (and the x-naut) to risk
human life.
